After Ryan Loft scored for the visitors early in the second half, the Millers equalized through goals from Mallik Wilks and Hakeem Odoffin.
It took some time for the game to pick up pace, and the first opportunity came for Rotherham when Joe Rafferty set up Wilks, but Danny Andrew made a crucial block.
Just before half-time, Odoffin missed connecting with Wilks' cross.
Cambridge surprised everyone by taking the lead in the 49th minute when Loft scored after receiving a pass from Josh Stokes.
Rotherham fought back and leveled the score in the 53rd minute when Wilks headed in Joe Powell's cross, which bounced in off the post.
Reece James posed a threat with a dangerous low cross that forced Jack Stevens to make a save.
Stevens made another important save shortly after when he stopped Louie Sibley's shot after Andre Green made a break down the left side.
The winning goal came for Rotherham in the 69th minute with a well-worked move down the left, finished off by Odoffin, who scored from Wilks' cross.
